APPOINTMENT POLICIES TO BE AWARE OF

APPOINTMENT TIMES

At Eternity Health Partners, our goal is to provide quality care in a timely manner. We have implemented a no show and cancellation policy which enables us to better utilize available appointments for our patients. The following policies is with regard to patients who fail to keep their scheduled office visit appointment.

Please be courteous and call our office promptly if you are unable to attend an appointment. Available appointments are in high demand and your early cancellation will give another person the possibility to have access to timely care.

LATE ARRIVALS

We are committed to being on time with patient’s appointments in order to prevent clients from waiting. Please arrive 15 - 30 minutes before your appointment time to allow for parking and navigation to our office at 533 E Micheltorena Street Suite 203, Santa Barbara, CA.

  • If you are less than 30 minutes late the amount of time you are late will be deducted from your allotted visit time.

  • If you are more than 30 minutes late you will forfeit your appointment time and be rescheduled for the next available opening.

  • If your appointment is determined forfeit you will be charged a “Rescheduling” fee of $50.00.

CANCELLATIONS

  • If you must cancel or re-schedule your appointment, please contact us at least 48 hours prior to your appointment without any penalty.

  • There will be a “No Show/Cancellation” fee of $50.00 for those who fail to show for their scheduled appointment or did not notify the office 48 hours prior to their scheduled appointment.

  • These fees are the sole responsibility of the patient who is scheduled for the allotted appointment.

  • To cancel or re-schedule your appointment, please contact our office at 805-608-4582. If you have any problems getting through, you can leave a message with your name, appointment date and cancellation reason or request for rescheduling.
